Community Health Foundation of Western and Central New York (CHFWCNY)
CHFWCNY is an independent private foundation serving the Western New York counties of Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, and Wyoming; and the Central New York counties of Cayuga, Cortland, Herkimer, Madison, Oneida, Onondaga, Oswego, and Tompkins. The Foundation invests in, and partners with, organizations and communities in innovative ways to improve health and healthcare for the vulnerable and underserved, particularly as it relates to frail elders and children from communities of poverty.
