Western New York’s P² Collaborative was selected as one of 15 communities throughout the country to take part in Aligning Forces for Quality, RWJF’s signature effort to spearhead health-quality reforms.

Amount Support Activities
Aligning Forces for Quality (AF4Q): The Regional Market Project  02/07-04/08   $600,000 This initiative brings an unprecedented commitment of resources, expertise and training to turn proven health care reforms into real results at the community level. The AF4Q initiative focuses on the full continuum of health care delivery—the care provided in doctors’ offices, clinics and hospitals, and the support provided in the community. It also emphasizes reducing racial and ethnic disparities in care and strengthening nursing’s role in improving quality.
It advances three interrelated reforms that experts believe are essential to improving health care quality:
  • Performance measurement and public reporting
  • Consumer engagement
  • Quality improvement
Special Use Grant 09/07-12/07   $25,000 Funding was used for:
  • Additional staff hours to assist with the coordination and implementation of strategies in the consumer engagement component of AF4Q
  • Retain a business consultant to facilitate the development of a sustainable business plan for P² which includes development of additional quality improvement initiatives for the WNY region
  • Retain faculty from the Center for Health and Social Research at Buffalo State College to review a consumer survey that was developed as part of the concept mapping process for cultural and literacy level appropriateness for intended audiences
Initial Communications Grant: Implementing, Enhancing & Supporting Existing WNY AF4Q Communications Plan  05/08-05/09   $50,000   Funding to support activities that will promote P²'s abilities to achieve progress in quality improvement and public reporting. Activities included
  • Hiring communications experts to manage all communication needs and promote AF4Q programming in Western New York.
  • Purchase paid advertising to promote P²'s AF4Q programs and resources including the diabetes resource guide, My Health Counts programs on public television, the P² website and the P² annual conference.
Aligning Forces for Quality: The Regional Market Project  05/08-04/11  $1,000,000  This initiative brings an unprecedented commitment of resources, expertise and training to turn proven health care reforms into real results at the community level. The AF4Q initiative focuses on the full continuum of health care delivery—the care provided in doctors’ offices, clinics and hospitals, and the support provided in the community. It also emphasizes reducing racial and ethnic disparities in care and strengthening nursing’s role in improving quality. Supplemental Communications Grant: Implementing, Enhancing & Supporting Existing WNY AF4Q Communications Plan 04/09-1/10   $50,000 Funding from this grant supports efforts to implement the existing communication plan as well as establishing new activities that will enhance it.
Specific activities include:
  • Managing media relations to promote and brand P² and AF4Q.
  • Develop and lead implementation of a strategic communications plan to promote the understanding of public reporting and prepare the community for public reporting.
  • Develop and deliver presentations to key audiences to educate them about the resources available from P²’s AF4Q programs.
  • Work with P²’s AF4Q team to recruit consumers to become active in P²’s Consumer Advisory and Leadership Teams.
  • Work with the WNY Health Equity Group to promote the elimination of health disparities among community health providers, hospitals, doctors, etc.
  • Oversee paid media activities to promote diabetes education among key audiences.
  • Manage internal communications related to AF4Q programs.
  • Translate and print a Spanish-language version of the diabetes resource guide that was developed with the assistance of the first AF4Q Communications Grant.
