Community Health Improvement
At the P² Collaborative of Western New York, we recognize that in order to improve health outcomes, we have to look at more than just health care. Our health is impacted by our behaviors, social and economic factors and the physical environment where we live and work. We take a broad perspective to improve the overall health of communities across the region by providing facilitation services, data support and by leveraging partnerships.
Through support from New York State Department of Health as a regional technical assistance provider, P² Collaborative has supported all eight WNY counties since early 2013 with their Community Health Assessments (CHA), Community Health Improvement Plans (CHIP), and hospital Community Service Plans (CSP). As a neutral convener, we have been able to support unprecedented levels of collaboration and resource-sharing across county and hospital partners.
The P² Collaborative is well-positioned to continue to serve in this role as support through the NYSDOH becomes available for other population health improvement initiatives in the region. This will open the door to more partnerships that will have a broad impact on the health of Western New York.
