P2 Collaborative

The P2 Collaborative’s second annual conference moves the people in our community towards a healthier future. This is our mission. Innovation is one path to a healthier community. However, innovation, which creates new movements in health care, can be difficult to implement and new systems and techniques can even stall in the communities where they begin. With this in mind, a key part of the 2008 conference will address the spread of health care innovations.

In an effort to improve the quality of health care of people in Western New York this conference will:

  • Highlight the importance of partnerships between people and their health care providers
  • Offer providers innovative health care models and quality improvement tools
  • Motivate people to take charge of their health and health care needs

Key Elements of the Conference:

Even though the efforts of the P2 Collaborative’s many partners is evolving constantly, the work remains committed to specific elements—bringing together those who receive health care, those who pay for health care, and those who provide health care. The breadth and depth of knowledge from experts invited to speak at the 2008 conference will engage those in attendance on the following topics:

Regional Quality Improvement: Efforts to develop continued community-wide capabilities to improve and sustain patient-centered, equitable and high-quality care in the region.

Consumer Engagement: Efforts to help people take an active role in their own health care, from understanding conditions and available treatments, to seeking out and making decisions based on information about health care providers, to understanding improvements in care.

Communication: Efforts to learn about, discuss, and fully understand communication options around patient self-management, partnering with providers and healthy lifestyles.

Collaboration: Efforts to work together; the conference will focus on successful health care collaborations around the country that dramatically improved quality and saved money.
