Area primary care physicians have the opportunity to access an on-line private portal to review their quality metrics of 19 HEDIS measures. The project is funded through the Western New York Quality Measurement Collaborative, comprised of the three area commercial health plans.

Aligning Forces for Quality

Aligning Forces for Quality (AF4Q) is the Robert Wood Johnson Foundation’s (RWJF) signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities and provide models for national reform. The P² Collaborative is one of 16 AF4Q communities in the nation.
