P2 Seeks a Manager of Clinical Care Transitions
SUMMARY OF JOB POSTING
The P2 Collaborative of Western New York (P2) is in need of a Manager of Clinical Care Transitions to handle project implementation of the WNY Rural Community-Based Care Transitions Program (CCTP) and manage the clinical aspects of other major projects within the P2 organization as needed. The Manager of Clinical Care Transitions will coordinate program implementation and operations for the CCTP project including collection, consolidation and reporting of data for the multi-county program in a manner than meets P2, regional partner’s, and funder’s expectations. The Manager of Clinical Care Transitions works under the direction of the Director of Regional Quality Improvement of P2.
ESSENTIAL FUNCTIONS
- Works to implement activities necessary to support contract and project objectives for the CCTP project
- Represents the P2 Collaborative as the regional community based organization (CBO) for CCTP
- Facilitates the partnership with 10 hospitals and eight community based organizations in locations across WNY to implement an evidence based program designed to improve care transitions
- Convenes and facilitates the Operations Committee meetings and any other meetings of partners to support effective operations and meet P2, CMS and other partner expectations
- Manages the development and maintenance of a project management website ensuring that the information needed by partner organizations is up-to-date
- Oversees the collection and distribution of information and communication materials necessary to support local and regional operations and support continuous improvement through implementation of the Care Transitions Intervention.
- Oversees the data collection process between P2 and community partners assuring accurate reporting and the meeting of all project deadlines
- Manages the tracking system and reports identified data for oversight of local operations for quality, model fidelity, efficiency and results
- Conducts CMS required post intervention survey
- Oversees the data aggregation and prepares program reports based upon criteria determined by CMS
- Oversees the invoicing process with local CBOs, including payments and payment requests for local CBOs
- Manages clinical aspects of other P2 projects including Improving Quality Improvement – Hospital and Transforming Care at the Bedside (TCAB) initiative
- Develops timetables and reporting structures in support of contract objectives for clinical aspects of all assigned projects and assists with communicating progress on contract deliverables for all assigned projects
- Develops tracking systems in support of clinical aspects of all P2 projects, as required
- Attends meetings for various initiatives within the P2 organization, as needed

REQUIREMENTS/QUALIFICATIONS

Minimum Bachelors level Nursing or other Clinical degree

At least 3 years of experience with grant and/or contract management
-
Familiarity with all major computer programs including excel, powerpoint, project manager software, sharepoint and access or other database programs
-
Demonstrated ability to interact with a wide range of personnel
-
Excellent written and verbal communication skills
-
Ability to handle many priorities simultaneously
