Welcome
Kevin Donovan, Chair-elect of the P² Collaborative of WNY convened the meeting at approximately 10:10 am. Shelley Hirshberg, Executive Director of the P² Collaborative of WNY requested that everyone introduce themselves.

Update: RWJF Aligning Forces for Quality
Marcia Wilson, PhD, MBA and Fatema Salam, MPH, Senior Research Scientists from The Center for Health Care Quality, The George Washington University, School of Public Health and Health Services, National Program Office for the RWJF Aligning Forces for Quality were present to give updates and answer questions.

Collaborative Updates
Before the group heard from representatives from the various initiatives P² is involved with, Shelley Hirshberg passed along some news & announcements.

The WNY Cancer Coalition and the P² Collaborative of Western New York have signed a Memorandum of Agreement. The WNY Cancer Coalition will become the “Cancer Arm of P2,” taking the lead in education, access, research and policy-making.
A Healthy Lifestyles initiative has been convened. The coalition has met several times. They are currently working on a strategic plan targeted for completion by the end of March. If you are interested in being part of this initiative, please contact Dawn Tierney at dawn@p2wny.org.
The 2009 P² Annual Conference will be held at the Buffalo Niagara Convention Center on September 24th & 25th. Planning for this conference is currently underway and we would like your input. Please take a moment to complete the conference survey and return it to Dawn Tierney at dawn@p2wny.org.

Performance Measurement
Raghu Ram, MD, and Richard Vienne, Jr, DO, representatives from the Western New York Quality Measures Collaborative (WNYQMC) gave a presentation updating the Collaborative on progress of the collaborative. The WNYQMC has been working with the three area health plans to aggregate quality measures and create a single quality report for the primary care physicians in WNY. The three health plans have submitted their data to ViPS, a third party data aggregator that has been contracted to consolidate the data. By the middle of February, the first report will be available for review by the WNYQMC Physician Advisory Work Group. The data is through 2007 and will be a baseline. By the end of the first quarter of 2009, a second report will be available for data through 2008. These reports will be available to physicians through a password protected portal created by Clinical Support Services. After reviewing the data, physicians’ will be asked to complete a short survey. Regional Quality Improvement
Ann Monroe, President of the Community Health Foundation of Western & Central New York, reported on efforts in regional quality improvement. Sheila Kee, Associate Director of the New York State Department of Health Western Region Office, recently met with P² to present regional data by prevention quality indicators available by zip code. It was striking how chronic conditions could vary from one zip code to the next, indicating that in fact one size does not fit all. Regional quality improvement is being addressed by two strategies:

Creating a common language across the region – In February, the P² Collaborative and Community Health Foundation of Western & Central New York are launching two two-day quality improvement training sessions. The individuals attending these training sessions will learn a common language, as well as common techniques and strategies for addressing quality improvement.
Reduce Disparities Among and Between Populations and Regions – Major disparities exist between race, ethnicity, language, etc. A work group, Health Equity in WNY Work Group, has been convened to address some of these issues. One meeting has been held, at which a great deal of interest was expressed. In late February, the group will be reconvening to create a vision as to what the group will be addressing. Consumer Engagement
Tracy Sawicki, the Consumer Engagement Team Leader of the P² Collaborative of Western New York, reported on the progress P² has been making on the consumer engagement front.

Consumer Principles – The National Partnership for Women & Families and the P² Collaborative are working to train people in WNY on Consumer Principles on Measurement and Public Reporting. Please take a moment to read through these principles. These principles help educate us on how to best translate the information to consumers in a relevant and usable fashion. Maine, a RWJ AF4Q community, has used these principles successfully. (Consumer Principles - Click Here)
Consumer Engagement Associates (CEAs) – Three CEA’s are currently on board: Christi Hayden with Lake Plains Community Care Network covering Genesee, Orleans and Wyoming Counties; Kerry Mihalko with the Chautauqua County Health Network is covering the Southern Tier; and Daryl Rasuli is working within the City of Buffalo, focusing in particular in zip code 14215. They are working to create a “web-of-support” to aid individuals after they leave the doctor’s office.
Stanford Chronic Care Model – Partnering with Independent Health to bring master trainers to the area to offer this four day intensive training session.
National Partnership for Women and Families: Empowerment Training – On March 5th & 6th, Christy Woods from the National Partnership for Women and Families will be holding Patient Empowerment Training sessions throughout the Western New York area. Patient advocates will be brought together to begin a conversation on the quality of care patients should expect and how to use public reporting to educate consumers. Diabetes Resource Guide – Has gone to print and is now available. A PDF version is available on the ThinkBright and P²Collaborative of Western New York websites. The Diabetes educators in WNY are now creating a distribution plan.

My Health Counts – Shows 2 and 3
Christy May and Pam Johnson, PhD, from WNED, presented on the progress of the My Health Counts series. Program Two which features Doriane Miller and Trissa Torres, MD, MSPH, FACPM, focusing on Patient Self Management is currently in editing and tentatively set to premier on April 1, 2009. Work has begun on Program 3 in the series, Getting Quality Care. Christy May and producers are currently looking to identify personal stories. If you have any suggestions, please forward complete suggestion form and return to Christy May at cmay@wned.org. In addition to the programs and public service announcements, WNED has recently launched a My Health Counts website to accompany the programs. Please take a moment to check it out.

WNY Cigarette Initiative
Jim Marshall, PhD, from Cancer Prevention & Population Science of Roswell Park Cancer Institute, presented on a new initiative to urge pharmacies to stop selling tobacco products. Jim Marshall, Tony Billittier and Shelley Hirshberg recently met with Commissioner Daines to discuss ideas. Commissioner Daines recommended using soft pressure tactics, such as education, survey of pharmacies with the creation of an “honor roll” and “dis-honor roll,” placement of notices regarding harms of tobacco in pharmacies, etc. P² has committed to offering support to this cause.
