P² has been leading an End of Life Planning initiative over the course of the past few months dubbed the Quality Living Coalition. The Quality Living Coalition is a multi-stakeholder, regional collaboration that will lead efforts to ensure that WNYers discuss their quality of life values and choices, as well as related care issues with loved ones. The list of current members can be found below.
P², along with the Quality Living Coalition and other national, state, and community organizations, is highlighting the importance of advance healthcare decision-making—an effort that has culminated in the formal designation of April 16 as National Healthcare Decisions Day (NHDD).
NHDD organizers are asking interested organizations and individuals throughout the country to help raise awareness about the importance of advance care planning on this special day—and throughout the year. To help realize this goal, they have created a website with information and tools for the public at all ages and stages of life to talk about future healthcare decisions and execute written advance directives (healthcare power of attorney and living wills) in accordance with their applicable state laws.
The message for this year's NHDD is that "It always seems too early, until it's too late." We want to encourage people to share their wishes now with their loved ones, no matter if you're 20 years old or 80 years old. It's never too early to have this important conversation.
Visit CompassionAndSupport.org for NYS information on advance care planning. This website is the central resource for NYS ACP and MOLST resources. For information on The Conversation Project, and Death Over Dinner, check out our blog post, "Let's Talk About It."
Want to join in the conversation online? If you are on Twitter, join the National Healthcare Decisions Day Initiative for two different Tweet Chats on Wednesday, April 13 at 9:00 and 12:00 ET. Use #NHDD and join the discussion.
NHDD shares a story that speaks to many people's experiences when a loved one dies without having had the conversation. JoAnn shares her story about the importance of having advance care planning discussions, sharing your wishes with your family & loved ones, and completing a Health Care Proxy.
