The P² Collaborative’s mission is to align resources to measurably improve health and healthcare delivery, increase access to care, and reduce health disparities in Western New York.
The Project Manager will provide coordination and oversight of the implementation of Linking Interventions for Total Population Health (LIFT) in four counties of Western New York that participate in this program, and coordinate with other staff, other programs and other partners on program execution. Specifically, the Project Manager will focus on overall program evaluation efforts within 3 distinct categories referred to as “buckets” (1) traditional clinical prevention (2) innovative clinical prevention, and (3) total population or community-wide prevention (working with Buffalo State College Institute for Community Health Promotion).
This is a grant-funded position with a grant period that ends in January 2019.
Limited, local Western New York area and occasional conferences. Mileage, tolls and parking reimbursement are provided.

According to the 2008 End-of-Life Care Survey of Upstate New Yorkers, nearly nine of ten local adults said it is important to have someone close to them making medical decisions for them if they were to have an irreversible terminal condition and were unable to make these decisions. Yet less than half have designated a spokesperson to assure their wishes are carried out. According to a report by the Centers for Disease Control and Prevention, nationally, approximately 70% of the population does not have an advanced care plan.

Let's see if we can create a viral movement that changes the way people think about advance care planning. We're officially challenging everyone who receives this to a dare.
