P2 Collaborative

Welcome to the Western New York Community Health Planning Institute (WNYCHPI), the planning arm of the P² Collaborative of Western New York (). We are a not-for-profit initiative dedicated to improving the health of (and health care in) our community. 

Our vision is to coordinate a collaborative, ongoing, sustainable, region-wide health planning capacity within the eight counties of Western New York (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyoming Counties), as well as align and provide supportive tools to assist communities and their stakeholders with their heath-planning efforts. 

The New York State HEAL 9 grant provided funding to establish a planning infrastructure (including this website) in an effort to serve as a community health planning resource that:

• provides a Community Health Dashboard
• recognizes the work of the community
• informs the community about our efforts

…as well as supports the overall objectives:

⇒improve the health of our community and positively impact quality and affordability of care

⇒ensure that WNY is recognized as a leader and a model for community health and health care 

Throughout this website, you may learn about our initiatives, health planning efforts and the valuable work being conducted by many community partners and stakeholders. We are also pleased to share with you information about planning models, best practices and resources, as well as the data that will be driving the improvements.
