April 16th is National Healthcare Decisions Day (NHDD), which focuses on making future healthcare decisions known to family, friends, and healthcare providers. As part of NHDD, we will help host a series of events.
We will launch a social media campaign to encourage you and your family and friends to share your wishes and complete your advance directive. Check out the Conversation Project’s website for their starter kit for tips on how to have the conversation. To participate, starting on April 8th, complete the forms on our website and pick your proxy. Once you’ve picked your proxy, take a picture together and tag us at @P2WNY to be entered to win a restaurant gift card (value of $50)! We will select three winners and announce them on Facebook on April 22nd.
Engaging Patients in Advance Care Planning Conference
Join us on April 8th for a half day conference, How to Make it Happen: Engaging Patients in Advance Care Planning, which continues the discussion from the September 2016 event on having meaningful conversations on advanced care planning for healthcare professionals. We are excited to offer mini sessions around psycho-social, spiritual, and situational aspects of advanced care planning.
